Community Action is the Collaborative Applicant, HMIS Lead, and CoC Lead for the Three County Continuum of Care (MA-507)

The Collaborative Applicant is the agency designated by the Continuum of Care (CoC) to submit the CoC Registration, CoC Consolidated Application, and apply for CoC Planning Funds. As the collaborative applicant, Community Action Pioneer Valley is responsible for submitting applications for Continuum of Care funding on the behalf of the continuum and agencies which receive Continuum of Care funding. The HMIS (Homeless Management Information System) Lead is the agency responsible for successfully completing applicable HUD reporting requirements, developing all plans, policies and procedures for review and approval by the CoC. The HMIS lead also executes all HMIS Partnership Agreements with each contributing HMIS organization, ensures that each HMIS user has signed an HMIS Code of Ethics, manages the system on a day-to-day basis, and provides technical support and training to users.
